Investigation Summary
An employee's job duties included taking the temperature of all employees and personnel who entered the facility through the front entrance. The employee would use an infrared thermometer to take each person's temperature and would ask them a series of questions about COVID-19 signs and symptoms. The employee's other duties included faxing documents, filing papers, and answering the phone. The employee tested positive for COVID-19 on December 4, 2020, during routine testing at the workplace. On December 11, 2020, the employee was admitted to the hospital and died from COVID-19 on January 4, 2021.
