Investigation Summary
At 11:50 p.m. on January 4, 2021, Employee #1, employed by a warehouse company, was assisting another employee unload freight from a trailer. The freight was awkward, so they used a strap wrapped around the forklift mast to pull a pallet from the truck. The strap became wedged into the mast, preventing the forklift blades from lowering. In an attempt to free the strap, the employee climbed on top of the forklift blades. Using a crowbar, the employee freed a piece of metal, which caused the mast and blades to drop. The employee had his left pinkie resting on the mast when it came down, resulting in the tip of his pinkie being sliced off. The employee was transported to the hospital, where the finger could not be reattached. The injury resulted in the amputation of the left pinkie finger. The employee was hospitalized.
