Investigation Summary
At approximately 4:15 p.m. on December 8, 2020, Employee #1 was setting up a Borghi brush-making machine for the next run cycle. During the setup, the drill became stuck in the table. Subsequent attempts to raise the drill resulted in the buildup of stored energy in the drill spindle due to the cam turning without movement in the spindle arm, holding the tension springs at full load. When the employee grabbed the drill shroud, the drill bit became free, causing the drill shroud to rise rapidly. This motion compressed the tip of the employee's middle finger on the left hand between the drill shroud and the base of the drill spindle housing, resulting in a partial fingertip amputation. The employee was treated without hospitalization.
