On December 9, 2020, the employee reported his illness to his employer. On December 15, 2020, the employee was hospitalized and tested for COVID-19. On December 17, 2020, the employee received a positive test result. The employee died on December 26, 2020. The employee had worked with a coworker who left work on December 7, 2020, and tested positive for COVID-19. At the time of his illness, none of the employee's family members tested positive for COVID-19. The employer did not have an effective COVID-19 Prevention Program in place.
