STRUCK BY TRAIN WHILE OPERATING SWEEPER

ON JUNE 15, 1984, AT APPROXIMATELY 11:30 AM, EMPLOYEE #1 WAS OPERATING AN OLATHE SWEEPER, MODEL 48 HL. HE WAS STRUCK BY A TRAIN WHILE CROSSING THE TRACKS. THE TRACKS CROSS OVER A PRIVATE ROAD. THERE WERE NO WARNING SIGNS POSTED. THE EMPLOYEE EITHER DID NOT SEE THE TRAIN APPROACHING OR HE TRIED TO BEAT THE TRAIN ACROSS THE TRACKS. EMPLOYEE #1 HAD BEEN INSTRUCTED IN THE OPERATION OF THE SWEEPER. THE SWEEPER WAS IN A SAFE OPERATING CONDITION.
