Employee with COVID-19 infection dies

Employee #1 was working at a residential care facility. She was an assistant residential manager of a residential group home. The facility was a group home with three adult residents who were developmentally or intellectually disabled. There were sixteen employees who staffed the home around the clock. All three residents and six of the employees were diagnosed as COVID-19-positive during a November 2020 outbreak. On or about December 6, 2020, Employee #1 died of COVID-19. The employer did not report the fatality to OSHA within eight hours of being informed by her family.
