Investigation Summary
On November 29, 2020, an employee who worked as a Cabler at a wire manufacturing facility died from COVID-19. The facility produced wire/cables for the aerospace industry. The employee ran multiple machines while performing his duties. He walked each morning to the Quality Assurance (QA) Laboratory where he took breaks and socialized with coworkers. The employee was known to only travel from home to work and back. On or about November 15, 2020, Coworker #1, who worked in the QA Lab, called Coworker #2 (Production Manager) to provide notification that a family member tested positive for COVID-19. That following Monday, November 16, 2020, Coworker #1 did not report to work since Coworker #2 ordered him to stay home until all members of his household tested negative for COVID-19 in accordance with company policy. Coworker #2 conducted contact tracing to determine who Coworker #1 had been in close contact with. On November 18, 2020, Coworker #2 also notified the QA Lab Staff to get tested. On November 17, 2020, Coworker #3 who worked in the QA Lab had left early due to a family emergency. Coworker #4, who also worked in the QA Lab, was on vacation that week. On November 18, 2020, Coworker #3 went to get tested after Coworker #2 notified her of the situation that same morning. The employee did not return to work pending her results, which she received on November 20, 2020. On that same date, Coworker #2 ordered the entire floor to shut down and all employees sent to a testing center where they received the results that same day. Out of the 45 employees tested, only the employee received a positive test result. The employee was sent home to quarantine but did not return to work. The employee continued to worsen at home until November 15, 2020, when his daughter took him to the hospital where he remained until his death. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative.
