Investigation Summary

An employee had worked as a Patient Care Associate general medical and surgical hospital. October 28, 2020, was the employee's last day of work. On November 6, 2020, the employee tested positive for COVID-19. On November 15, 2020, the employee was released to return to work but called in sick. The employee was scheduled to work on November 22, 2020, but did not show up and did not call into work. On December 3, 2020, the employer learned that the employee had died. Unconfirmed reports indicated that a family member found the employee dead on November 26, 2020. The death is potentially related to COVID0-19, but this was not confirmed at the time the employer reported the death. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative.
