Investigation Summary

Investigation Nr: 131659.015
Event: 11/17/2020
Employee with COVID-19 infection dies

At 8:56 p.m. on November 17, 2020, an employee (Registered Nurse) who had been providing direct patient care tested positive for a COVID-19 infection. On November 27, 2020, the employee was hospitalized and on December 8, 2020, the employee died. The employee had been employed as an RN at the facility for approximately 9 years. Positive COVID-19 test results were returned on November 17, 2020. The facility had a designated COVID-19 unit for patients who tested positive, but at the time the employee became ill, he was working at a non-COVID-19 unit. The COVID-19 unit was separated from the other areas of the facility by a plastic barrier. The employer conducted entry screening and mid-shift checks for COVID-19 symptoms. The employer also provided testing every Monday and Thursday for employees. The employer also allowed employees to seek outside testing if they were not at work and asked that employees provide positive test results to the employer followed by quarantining. The employee was required to wear a KN95 or N95 mask, but the employer is not sure which one the employee wore and whether the employee wore a new or used respirator. The employer did allow employees to reuse respirators unless they were soiled or wet. Employees were provided paper bags for respirator storage. On November 25, 2020, the entire facility was classified as COVID-19 positive when nearly all patients contracted the virus.
