Investigation Summary
At 9:00 a.m. on December 4, 2020, an employee who worked as a Pharmacist for a Medical Center died from COVID-19. The employee had one-on-one limited contact with patients during as part of his job duties. The employee was provided with personal protective equopment (PPE) including gloves and a respirator and a barrier was installed at the Pharmacy counter for customer interactions. The employee contracted COVID-19 and was hospitalized after diagnosis but did not recover. Despite the controls in place, the employer did not have an effective COVID-19 Prevention Program.
