Investigation Summary

At approximately 1:00 p.m. on December 1, 2020, an employee was at home and fell, resulting in the employee being hospitalized. The employee's last day of work was November 19, 2020. The employee was on a regularly scheduled weekend from November 20, 2020, through November 22, 2020. On November 23, 2020, the employee did not report for work and the employer called the employee. The employee informed the employer that he was not feeling well. The employer instructed the employee to stay home and await the results of the COVID-19 test performed. On December 1, 2020, the employee died from Acute Hypoxic Respiratory Failure resulting from novel coronavirus (SARS-CoV-2) SARS-CoV-2.
