Investigation Summary

At 2:09 p.m. on November 30, 2020, an employee who worked as a Substance Abuse Counselor at a Residential Mental Health and Substance Abuse Facility died from COVID-19. The employee started work at the facility on November 2, 2020. The facility experienced a COVID-19 outbreak at which time the employee was exposed to COVID-19 positive trainer. The employee developed symptoms a few days later. The employee may have contracted COVID-19 from a community contact or a contact at work since both were a possibility. On November 17, 2020, the employee was hospitalized and released on the same day with a pneumonia diagnosis. He was then readmitted on November 19, 2020, with a positive COVID-19 test and diagnosis prior to his death. The facility had a COVID-19 protection program in place and appeared to have been utilizing all available precautions to prevent spread of the disease.
