Investigation Summary
At 6:45 a.m. on November 23, 2020, Employee #1 was operating a Skinner Townsend 7600 machine at a processing facility. During operation, their right hand's index and middle fingers were caught in the machine's contact points, resulting in lacerations and/or partial amputation.
