STRUCK BY FREE FALLING RAM IN PRESS CHUTE

ON AUGUST 7,1984, AT APPROXIMATELY 3:20 PM, EMPLOYEES #1 AND #2 WERE IN THE CHUTE SECTION (4' X 4' X 16') OF A FISHBURN PRESS. THEY WERE REMOVING EXCESS TOBACCO FROM THE CHUTE ONTO A CONVEYOR BELT. THE OVERSEER HAD JUST COMPLETED A REPAIR OF THE BANDER-STAMPER, AND WAS UNAWARE THAT THE TWO EMPLOYEES WERE IN THE CHUTE. HE ACTIVATED THE PRESS, CAUSING THE RAM TO DESCEND AND STRIKE EMPLOYEE #1. EMPLOYEE #2 YELLED, CAUSING THE OVERSEER TO HIT THE EMERGENCY STOP BUTTON. EMPLOYEE #1 WAS KILLED. EMPLOYEE #2 WAS NOT STRUCK.
