Investigation Summary
An employee was working as a nursing assistant at a nursing care or skilled nursing facility. She was providing care to COVID-19-positive residents in the secured dementia care unit. She was responsible for assisting the residents with all their daily life skills, such as bathing, dressing, eating, and using the toilet. The employee wore a N95 respirator, full face shield, gown, footies, and gloves while working directly with COVID-19-positive residents. The employee had not been fit-tested prior to being required to wear the N95 respirator for protection from SARS-CoV-2. Sometime between 7:30 a.m. and 11:00 a.m. on November 4, 2020, she began to feel achy and feverish. The employee reported her symptoms to her employer, and a rapid COVID-19 test was performed at 11:00 a.m. The result was positive. The employee was sent home to quarantine. At 12:59 p.m. on November 8, 2020, the employee's family members went to her residence check on her. They found her dead. The cause of death was cardiopulmonary arrest due to infection with COVID-19. In the ten days prior to the onset of her symptoms, the employee had worked seven shifts as a nursing assistant in the dementia care unit. That unit had been designated as a COVID unit once an outbreak in the facility began the week of October 11, 2020.
