Investigation Summary

At 8:00 a.m. on October 30, 2020, an employee who worked as a Warehouse Worker for a third-party logistics provider was hospitalized with COVID-19. On November 4, 2020, the employee died due to complications related to COVID-19. The employer provides transportation, warehousing and distribution for clients. Prior to his death, the employee had been engaged in cleaning in the warehouse. A coworker who had also been a Warehouse Worker engaged in preparing products to be shipped from the warehouse was also reported hospitalized with COVID-19. The employee and the coworker had the same immediate supervisor who was the General Manager. There was no evidence of a workplace exposure found for either employee. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are also not included in the narrative.
