Investigation Summary
An employee was working in law enforcement for a township. He was a police officer. His last day of work was October 22, 2020. He tested positive for COVID-19 on October 28, 2020. He was hospitalized. At 8:00 p.m. on November 4, 2020, he died due to complications of COVID-19, according to his death certificate. He had prior close contact with a coworker who eventually was diagnosed with COVID-19 on October 24, 2020. The employee was one of seven work-related illness cases of COVID-19 cases that arose at the same employer. One employee tested positive for COVID-19 on October 27, 2020. Two other employees tested positive for COVID-19 on October 28, 2020. An additional two employees tested positive for COVID-19 on October 29, 2020. Two more employees tested positive for COVID-19 on October 31, 2020, and on November 1, 2020. All seven of these employees worked for the police department and missed days away from work. These seven work-related illnesses were not recorded on the required forms, or their equivalent, within seven calendar days of the time that the employer knew of the information.
