Employee injured in fall from roof

At 2:00 p.m. on April 25, 1989, Employee #1 was installing skylights in the metal roof of a building. He and two coworkers had placed a skylight and then positioned a 10 ft long by 39 in. wide metal roof panel in place. Employee #1 had not screwed the panel down, and it slipped when he stepped on it. Both he and the panel fell 22 ft to the concrete floor. Employee #1 was hospitalized with multiple fractures.
