Investigation Summary

An employee was working for a manufacturer of conveyors and conveying equipment. He worked in the engineering department. His workstation was at the end of a row of cubicles in which five cubicles were located along an exterior wall with windows, and five cubicles were arranged right across an aisle. He and his coworkers worked in cubicles that were squares between 1.8 and 2.1 meters wide (6 and 7 feet wide), with walls that were 1.8 meters (6 feet) high. While workers were working at their desks, they were not required to wear masks. At times, employees would be conversing in these areas as they were spaced less than 1.8 meters (6 feet) apart. Several workers in the timeframe of a week experienced signs and symptoms of COVID-19 and ultimately tested positive for it. It could not be determined if the employee's illness was work-related or part of a community spread. As the investigation was opened, the employer was not aware that the employee had even tested positive for COVID-19. At 12:55 p.m. on October 7, 2020, the employee died.
