Investigation Summary
Several employees were working for a site cleanup contractor. The employer's primary business was providing water and flood recovery, fire and smoke recovery, mold remediation, lead and asbestos abatement, biohazard mitigation, and infection prevention and disinfection. Since March 2020, the employer had also provided COVID-19 disinfection services. Around the time of the incidents, the employer was performing site cleanups on multiple sites. The two main job sites where most of the employees who contracted COVID-19 worked were in a major city. Eleven employees worked as technicians, lead technicians, and team leaders. They had their last days of work in mid- to late-September 2020. Employee #1 worked as a technician. His last day at work was September 18, 2020. He went to the hospital on September 25, 2020, and he was hospitalized. The following day, he tested positive for COVID 19. On October 24, 2020, he died. One of the other ten employees was hospitalized for thirty days for COVID-19 before returning to work. The other nine employees self-quarantined at home before returning to work. Some of the employees were related. Others carpooled together. Fifteen other employees were tested and found negative for COVID-19 around the same time. The employer worked with the city's public health inspector after these incidents. The employer provided PPE including facemasks as needed. The employer has provided training on COVID-19. The employer has required employees to report symptoms of COVID-19 and not to report to work if they have symptoms. No accident-related citations were issued, as no violations of Title 8 were found in relation to the cause of the incidents.
