Investigation Summary

Investigation Nr: 14557359
Event: 10/10/1984
Employee killed when struck by falling floor sections

At 3:00 p.m. on October 10, 1984, Employee #1 was installing rebar in soft cement. He was working under a steel frame that was to be the roof of a garage for the El Dorado Hotel when three sections of prestressed concrete flooring fell 20 ft from the steel frame. Employee #1 was killed. Two iron workers jumped from the falling roof to the fourth section and escaped injury. The frame was not supported in a way to prevent it from separating, and the outer column was supported only by a single horizontal beam at the 20 ft level. The flooring fell as a fourth section was being positioned by a crane.
