Investigation Summary
At 7:00 p.m. on August 6, 2020, Employee #1, a Registered Nurse, was hospitalized with pneumonia and COVID-19 symptoms. Employee #1 had shortness of breath and tachycardia with mild exertion and was diagnosed was COVID-19 related pneumonia. On July 24, 2020, Employee #1 was working on the telemetry floor of a hospital. The hospital had two telemetry units, one for COVID-19 floor and one non-COVID-19, which nurses float between. Employee #1 had performed a nasopharyngeal (NP) swab on a COVID-19 patient while wearing a level 2 surgical mask and on July 25, 2020, he cared for a patient who was reported positive for COVID-19 on July 30, 2020. On July 31, 2020, Employee #1 began to exhibit COVID-19 related symptoms such as body aches and a sore throat. On July 31, 2020, Employee #1 was sent home pending test results. On August 1, 2020, Employee #1 was tested and on August 3, 2020, the test was reported positive for COVID-19. On August 7, 2020, Employee #1 was discharged. On December 24, 2020, Employee #2, was hospitalized due to COVID-19 related symptoms. Employee #2 worked as an administrative staff employee in the inpatient staffing office. The staff were not exposed to patients but were exposed to nursing staff. On December 11, 2020, Employee #2 had an unplanned interaction with four coworkers in the break room in which the coworkers removed their face coverings to eat and failed to practice social distancing. On December 12, 2020, one of those coworkers tested positive for COVID-19. On December 15, 2020, Employee #2 began to feel ill and left work. On the December 16, 2020, Employee #2 was tested for COVID-19 and on December 18, 2020, the test results came back positive. On December 24, 2020, Employee #2's blood oxygen began to drop and she went to the ER where she was admitted. Employee #2 was treated with antibiotics and oxygen and later discharged (date unknown).
