Employee with COVID-19 infection dies

Employee # 1 worked during the two weeks prior to the onset of the symptoms on August 20, 2020. Employee # 1 contracted COVID-19 and was hospitalized on August 28, 2020. At 2:30 p.m. on September 6, 2020, Employee #1 died. His Employer did not report the hospitalization to the state OSHA and reported the fatality on September 8, 2020. The employer report shows three COVID-19 cases at the site during the previous 14 days of Employee # 1's onset of symptoms; two cases in a different shift and one case in the same shift as Employee # 1. According to the employer contact tracing, Employee # 1 was not identified as a close contact to any of the three cases.
