Investigation Summary
The employer implemented regular COVID-19 testing for staff and residents. Staff with positive results were sent home to quarantine. Residents who tested positive were isolated as a cohort. The only staff who worked with COVID-19 positive residents were designated COVID-19 staff. The employer required facemasks and PPE be worn at all times and provided training and regular testing. Employee #1 was tested in the first round of testing that began in early June and was positive. Employee #1 had symptoms and underlying health issues and self quarantined for two weeks. On June 16, 2020 employee #1's test came back negative and she returned to work and volunteered to work in the red zone as she had gone through COVID-19 and thought she was unlikely to catch it again. Employee #1's last day of work was July 26, 2020 and the next day she reported not feeling well and called in sick. Employee #1 was hospitalized on August 7, 2020, for three weeks. Employee #2 worked in the dietary department and started having symptoms during her weekend off and did not go into work with symptoms. On August 13, 2020, she was hospitalized and returned to work on September 16, 2020. Employee #3 had worked with both staff and residents who were positive for COVID-19 in the two weeks prior to her symptom onset. Employee #3 was a CNA whose duties included patient care, assisting with activities of daily living - dressing, feeding, showers, toileting, grooming, and ambulation.
