Investigation Summary

Investigation Nr: 129036.015
Event: 07/19/2020
Employee with COVID-19 infection dies

An employee was working for a poultry processor. He worked in the injector feeder department. On June 17, 2020, he was tested for COVID-19 at an onsite company-wide test held by his employer. He then worked from the 17th through the 19th of June, 2020. June 19, 2020, was his last day of work. On June 20, 2020, he was notified that he had tested positive. He was quarantined. On June 22, 2020, the employee was hospitalized. Somewhere in the timeline, he tested positive a second time. On July 19, 2020, he died of COVID-19. On that same day, his employer was made aware of the fatality. The employer did not report the fatality to OSHA until it was contacted by a compliance officer on August 19, 2020. The compliance officer had noted the employee's death in a review of the 300 logs kept by the employer.
