THE EMPLOYEE WAS WORKING FROM A LADDER WHICH WAS NOT TIED OFF OR SECURED FROM ACCIDENTAL DISPLACEMENT. THE LADDER SLIPPED AWAY FROM THE WALL, CAUSING THE EMPLOYEE TO FALL 20 FEET TO THE CONCRETE FLOOR, STRIKING HIS HEAD.
