Investigation Summary

Investigation Nr: 128547.015
Event: 07/29/2020
Employee with COVID-19 infection dies

An employee worked on a production line and was responsible for keeping the line stocked with product for coworkers packing orders on the line. The employee would come within six feet of other workers for brief moments when walking between assembly lines and when placing stock in bins on the line. The rate of proper use of face coverings was less than 100 percent and plastic barriers placed between employees working on the line were not installed in a manner to be a physical barrier between their level of the breathing zones while working within six feet of each other. Employees received 15 minute and 40 minute breaks at the same time and could take them inside in a designated breakroom or outside in their vehicle. The employee contracted COVID-19 and died at the hospital at 8:00 a.m. on July 29, 2020, due to respiratory distress and pneumonia.
