Investigation Summary
At 10:00 a.m. on July 30, 2020, an employee was operating a Milko-7 milling machine for the first time. The machine was set up by a coworker to mill a flat section onto a piece of round stock that was approximately 1 inch in diameter and 2 inches long. In the course of loading the part into the machine's pneumatic hold-down clamp, the employee's finger was caught in the V-block. The pinch point created as the V-block lowered onto the part was not guarded. The employee suffered a partial amputation to the end of her left index finger. The employee was treated but not hospitalized.
