Investigation Summary
At 1:00 a.m. on July 21, 2020, Employee #1, a Gas Tester employed by Tutor Perini / O & G A Joint Venture, was assisting with the movement of a steel key segment at the Constellation Launch Box, an underground subway station under construction. Employee #1 was performing gas testing when a key segment of the Starter Ring for the tunnel identified as BR-TBM fell off a Temporary Segment Feeder. Attempting to reposition the key segment using a Burke bar, Employee #1 became distracted and relaxed his grip. He heard a loud "boom" and realized that his fingers were trapped between the Burke bar and the Temporary Segment Feeder. Although Foreman #1 and General Foreman #1 heard Employee #1's cries for help and assisted him out of the site. Employee #1 was taken to Ronald Reagan UCLA Medical Center, arriving at approximately 1:05 a.m. and was treated for his injuries. Employee #1 was released from the hospital around 6 a.m. but later underwent a partial finger amputation at Downey Surgery Center on July 30, 2020. The accident was reported to Cal/OSHA on July 29, 2020, after the employer learned of the seriousness of Employee #1's injury. A DOSH Safety Engineer initiated an investigation on July 31, 2020.
