Investigation Summary

Investigation Nr: 128100.015
Event: 04/24/2020
Employee with COVID-19 infection dies

At approximately 12:00 a.m. on April 24, 2020, an employee returned to work on April 13, 2020, after being off while helping his spouse who just had their first child and told his supervisor that he was not sleeping because of the newborn. The employee was fatigued at work and his manager indicated he should get checked for COVID-19. The manager received a call from the employee on April 20 stating a positive test result. The manager alerted the Regional representative. The employee was told to track his symptoms and call the direct supervisor every day to report any symptoms for fourteen days. On all emails, the employee's temperature remained no higher than 99 and only reported a cough. The employee died on April 24, 2020 from complications from the virus.
