Investigation Summary

Employee #1 worked as a seasonal sanitation department supervisor for a fruit processing and canning facility. Employee #1's last day at work was June 26, 2020, and she was hospitalized on July 5, with COVID-19 related symptoms. Employee #1 died on July 20, 2020, due to complication of her infection. Employee #2 worked as a seasonal porch crew leader and her last day worked was July 16, 2020. Employee #2 called in sick on July 17, and on July 20, she was hospitalized due to complications of a COVID-19 infection. Employee #2 had a spouse that was also working for the employer and he was sent home immediately to quarantine for 14 days. The employer had a written COVID-19 prevention plan and employees received COVID-19 training. The employer conducted COVID-19 screenings, performed temperature checks with a scanning camera, provided and required face coverings, routinely cleaned and disinfected all frequently touched surfaces, conducted deep cleaning, developed non-punitive leave policies, performed post COVID -19 confirmation contact tracing, and required six foot social distancing of employee lines at clock-in machines. Sorting tables allowed 6 feet of physical distancing with additional plexiglass barriers. Lunch tables allowed 6 feet of separation between employees.
