Investigation Summary

Investigation Nr: 127906.015
Event: 06/18/2020
Employee with COVID-19 infection dies

The worksite is a general medical and surgical hospital. Employee #1 worked part time at this employer as a registered nurse, and also full-time at another hospital. On March 14, 2020, Employee #1 went out on medical leave and was admitted to the hospital on April 1, 2020 until May 29, 2020. The workplace had no COVID-19 patients identified prior to Employee #1's last day of work. The employers first COVID-19 case was admitted on March 15, 2020, and Employee #1 did not care for any COVID-19 patients at this workplace. On June 18, 2020, the employee died, and Employer #1 reported this information to the Cal/OSHAs Santa Ana District Office (SADO) on June 22, 2020 at about 4:09 pm. Information received during the inspection could not verify if this hospitalization was work-related with this employer, therefore there was no 8-hour reporting requirement required by current Cal/OSHA regulations. Employeer #1 told the Division that they called in this fatality to ensure that their reporting requirements were met, and that there was a verbal statement made that Employee #1 was positive for COVID-19, though no documentation regarding a diagnosis was received by the employer. DOSH initiated this inspection on June 23, 2020 with Employer #1 and Employer #2. Hospitalization due to COVID-19, though exposure cannot be substantiated at this workplace. Sars-Cov-2 virus and subsequent COVID-19 illness
