SERIAL #LTV- 154S. THE EMPLOYEE LOST FINGERNAILS OF INDEX AND MIDDLE FINGERS OF RIGHT HAND. THE EMPLOYEE HAD BEEN OPERATING HARRIS PRESSER FOR OVER 10 YEARS.
