Investigation Summary
At 11:00 a.m. on July 17, 2020, Employee #1 worked on a ward known to have regul ar Code Gray events. During Code Gray events, developmentally-delayed and other distressed COVID-19 patients, wander out of their isolation areas. Physical cont rol, restraint and assistance are often required to return them to their rooms. During the weeks prior to Employee # 1's illness, she helped in multiple Code Gr ay events. Face masks were required in the hospital halls outside the patient ro oms and N95s were required in COVID-19 rooms. Employee # 1 contracted COVID-19 a nd died from complications related to COVID-19, SARS-Co-V2.
