Investigation Summary
At 10:00 a.m. on June 18, 2020, an employee who had worked for a vocational rehabilitation services company died from COVID-19. The employee's last day of work was on May 25, 2020. The next day, on May 26, 2020, the employee reported flu-like symptoms and went to the hospital where he was admitted. On June 3, 2020 the employee was transferred to the intensive care unit (ICU) where he remained through June 15, 2020. The employer learned of the employee's death on June 19, 2020.
