STRUCK BY FORK LIFT

A FORKLIFT TRUCK WAS BEING USED TO UNLOAD PALLETS OF CHICKENS OFF A TRUCK AND ONTO A CONVEYOR. THE CHICKENS WERE THEN HUNG ON ANOTHER CONVEYOR FOR PROCESSING. THE EMPLOYEES HANGING THE CHICKENS WERE ON A BREAK. THE INJURED EMPLOYEE WALKED TO THE AREA AT THE END OF THE CONVEYOR WHERE THE LIFT TRUCK WAS PLACING THE PALLETS. HE WAS STRUCK BY THE PALLET AS THE OPERATOR WAS MAKING HIS SWING TO PLACE THE PALLET ON THE CONVEYOR. THE EQUIPMENT WAS A 1983 KD FORKLIFT, MODEL #T602 TCP-D, SERIAL NUMBER T6020058.
