Investigation Summary

Investigation Nr: 127269.015
Event: 06/22/2020
Employee with COVID-19 infection dies

An employee was working at a residential facility for people with intellectual and developmental disabilities. He was part of the direct support staff. Prior to March 13, 2020, he was responsible for bringing residents to the day program by bus and returning them at the end of the day. The day program was closed due to COVID-19. After March 20, 2020, he took food carts to and from the residents, helped them with their daily living needs, and removed trash. After April 1, 2020, he would knock and leave the food cart outside the door to specific rooms where COVID-19-positive residents were in quarantine. He also removed the trash left outside the door. He felt sick at home and called out for two days. On April 9, 2020, his employer was been notified of his illness. The employee went to the hospital. He died from complications of COVID-19, following an exposure to the coronavirus. On April 21, 2020, his employer was notified of his death. On April 25, 2020, the employer posted notice of the employee's death on its Facebook page. It did not notify OSHA until June 25, 2020. On April 1, 2020, the employer first learned of a symptomatic resident at the site. The employer made the first notation of COVID-19 symptoms in employees in the OSHA log on April 3, 2020.
