Investigation Summary
At 5:15 p.m. on June 10, 2020, an employee was removing dough from a dough-dividing machine and attempting to clear a photo eye, because the dough-dividing machine was malfunctioning. The employee got his hand caught in a shear point. The machine was not powered off, a safety switch had been by-passed, and another employee started the machine without knowing it was being worked on. It occurred toward the beginning of the second shift. The employee amputated three fingers and was hospitalized.
