Investigation Summary
On June 1, 2020, Employee # 1 died of COVID-19 related symptoms. According to the employer, Employee # 1 worked her last shift on May 1, 2020. On May 7, 2020, Employee # 1 called the employer's COVID-19 line to report symptoms, which she reported to have started on May 03, 2020. Employee # 1 was tested for COVID-19 on May 8, 2020 and the results came back on May 15, 2020, which were negative. She was tested again on May 21, 2020 and the results were positive for COVID-19. She continued to have symptoms until June 1, 2020 when she had a follow-up with a primary care physician. She was advised to go to the emergency room where she died.
