Investigation Summary

Investigation Nr: 128277.015
Event: 04/26/2020
Employee with COVID-19 infection dies

At 4:00 a.m. on April 26, 2020, Employee #1 worked with a coworker to provide assistance to an blind, intellectually challenged resident. Employee #1 and a coworker worked with the resident Monday through Friday between the hours of 3 pm-11 pm--this included administering medication, conducting G-tube feedings, providing meals, and ensuring that the resident remained calm and safe. Employees working directly with the resident, who was classified as health sensitive, were issued and required to wear cloth masks in the beginning of April. When the resident became ill in mid-April, Employee #1 and the coworker transported the resident to have COVID-19 testing and chest x-rays conducted. The coworker indicated that Employee #1 appeared ill and was coughing profusely on the day of transport. Employee #1 called out of work the next day due to feeling fatigued and a scheduled telehealth appointment with his primary care physician. Employee #1 was infected with COVID-19 and later died to the illness.
