Investigation Summary

At 10:30 p.m. on June 4, 2020, an employee who worked as a Respiratory Therapist at a rehabilitation hospital had reportedly died from COVID-19. The employee may have contracted COVID-19 from potential exposure to respiratory droplets while providing therapy to patients. On April 12, 2020, the employee had assisted a patient who she felt was a suspect COVID-19 patient. Two weeks prior to that date, the employee had developed COVID-19 related symptoms but tested negative. On April 14, 2020, the employee developed COVID-19 related symptoms again including a fever and body aches. On April 15, 2020, the employee tested positive and on April 16, 2020, she was admitted to the hospital. The employee had a history of asthma. On April 18, 2020, the employee was sent home where she remained for 5 hours, but returned to the hospital. On April 23, 2020, the employee was put on a ventilator. The employer was informed that the employee had died on June 4, 2020. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative.
