Event: 06/01/2020
Employee with COVID-19 infection dies

At 10:00 p.m. on June 1, 2020, an employee had been working at a plastics molding facility when he died from complications due to COVID-19. The employee was a temporary worker assigned to work at the facility by a staffing agency. On May 11, 2020, the employee called in sick. On May 12, 2020, the employee returned to work and worked through May 15, 2020, which was his last day of work. On May 15, 2020, a coworker called in sick and reported a positive COVID-19 test. The employer immediately quarantined the entire shift following the positive case and all of the employee's coworkers were instructed to get tested. The employee originally refused to get tested but relented and tested positive for COVID-19 on May 18, 2020. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative.
