Investigation Summary
An employee was working as a certified nursing assistant (CNA) in a skilled nursing facility. Several residents and possibly some of the other workers had tested positive for COVID-19. The employee contracted COVID-19. He was hospitalized, and he died on May 22, 2020. The facility had its first COVID-19 positive resident diagnosed on April 5, 2020. This was the first of 56 residents that were diagnosed with COVID-19 between April 5, 2020, and May 27, 2020.
