Investigation Summary
On April 24, 2020, Employee # 1, a physical therapy employee, began having symptoms of COVID-19 and was sent home from work. On April 25, 2020, the employee's mother found her unresponsive at home and had her brought to the hospital. The next day Employee # 1 was placed on a ventilator. On April 26th, Employee # 1 was removed from life support and she died on April 27, 2020.
