Investigation Summary

An employee was working for a firm that processed financial transactions and acted as a clearinghouse. At the site where he worked, the firm did commercial printing. He was the lead quality control coordinator. His shift ran from 8:00 a.m. to 5:00 p.m. He had been employed by the firm for 14 years. His last day worked, with a surgical mask worn, was March 27, 2020. On March 28, 2020, he had documented COVID-19 symptoms. He had a COVID-19 test on March 29, 2020. His symptoms were treated as those of pneumonia until the test results came back. He was admitted to the hospital on March 30, 2020. On April 1, 2020, the test result came back positive. He had contracted COVID-19. At 8:15 a.m. on April 17, 2020, he died. This was an OSHA-covered fatality. This investigation has inspection number 1475342. There were two other warehouse workers at this site who contracted COVID-19 and died. One died on April 5. That fatality had investigation number 126195 and inspection number 1475327. The other died on April 19. That fatality had investigation number 126197 and inspection number 1475337. They too were OSHA-covered fatalities.
