Investigation Summary

An employee was working for a firm that processed financial transactions and acted as a clearinghouse. At the site where he worked, the firm did commercial printing. He was the senior manager of production. His shift ran from 8: 00 a.m. to 5:00 p.m. He had been employed by the firm for 29 years. His last day worked was March 20, 2020. His symptoms started on March 21, 2020. He was tested on March 27, 2020 for COVID-19. The paperwork didn't have a specific date for a positive test. His family had taken a prom picture on March 14, 2020. One of the parents had tested positive for COVID-19 on March 16, 2020. Somewhere, the employee had contracted COVID-19. At 8:00 a.m. on April 19, 2020, he died. This was an OSHA-covered fatality. This investigation has inspection number 1475337. There were two other warehouse workers at this site who contracted COVID-19 and died. One died on April 5. That fatality had investigation number 126195 and inspection number 1475327. The other died on April 17. That fatality had investigation number 126198 and inspection number 1475342. They too were OSHA-covered fatalities.
