Investigation Summary
At 9:45 p.m. on May 6, 2020, an employee who had worked as a health aide at an assisted living facility died from complications related to COVID-19. The employee had been at home on quarantine following a COVID-19 positive test. The employee's spouse called 911 and the employee died before ambulance arrived. The employer did not have an effective COVID-19 Prevention Program or an adequate respiratory protection program in place.
