Investigation Summary

An employee was working as an infection control nurse at a veterans hospital. She worked in the facility's Community Living Center, where she faced potential exposure to COVID-19. Her time there was spent time training staff on infection control practices for COVID-19. She had also been responsible for fit-testing on site. She herself had not undergone a fit test or medical evaluation since 2015. The employee became symptomatic and was hospitalized approximately two weeks following potential exposure in the Community Living Center. Her last day of work was March 30, 2020. On May 19, 2020, she died of complications due to COVID-19. This was an OSHA-covered fatality. This investigation, number 126048, was associated with inspection number 1474791. There was another COVID-19-related fatality at this workplace. It had investigation number 126050 and inspection number 1474804.
