Investigation Summary
At approximately 4:45 p.m. on May 4, 2020, Employee #1 and Employee #2 provided direct care to an ill COVID-19 resident without respiratory protection, PPE, or appropriate infection control methods. The employees were exposed to the COVID-19 virus and became ill. The virus was transmitted from residents to employees, and they became ill too with COVID-19. Both employees were hospitalized and died as a result of COVID-19 illness.
