Investigation Summary
At 10:00 a.m. on May 6, 2020, Employee # 1 was working in a nursing unit in which multiple patients were leaving the facility for dialysis treatments up to three times a week. Many of the dialysis patients started demonstrating symptoms COVID-19. She and other employees were required to test the patients via a nasal swab for the COVID-19 virus without adequate personal protective equipment. She contracted the virus and died. Her death certificate stated the cause of death was cardiomyopathy and pneumonia due to COVID-19 Virus.
